At Community Hospice we take our mission of embracing patients and their families in their time of need very seriously. We are always exploring creative opportunities that will help reduce the stress that end of life care can sometimes bring.
Led by our Community Hospice Volunteer Department, our Tuck-In Program is designed to ensure that each of our patients, families and caregivers receive a friendly call once a week. A specially trained volunteer will call on Thursday, between the hours of 9:00 and 11:00 a.m. to check on supplies, medications and equipment. Our Tuck-In Program will help with anticipating patient, family and caregiver needs and filling any requests will relieve anxiety that you might have regarding medications, supplies or equipment required over a weekend.
If a patient/family need is identified that was not previously addressed during the patients nursing visit, our volunteer will relay the information immediately to the nurse or Manager for assistance. For additional information about our Tuck-In Program, call 209.578.6300.
