The Art of Caring Together

At CHHS, caring for patients and families is never the work of one person. It is the collective effort of nurses, physicians, social workers, chaplains, aides, volunteers, administrative teams, support staff, and countless others working together with one shared purpose: enhancing quality of life for all.

Our Associate Medical Directors, Dr. Miguel Hernandez and Dr. Nahera Adams, along with Medical Director Dr. Peter Lai, all echoed the same sentiment during recent conversations. While each brings unique expertise to patient care, none views hospice as an individual practice. Instead, they describe it as a collaborative art that depends on the strengths, compassion, and perspectives of every member of the team.

Hospice is grounded in medicine, but it is equally rooted in understanding people. Every patient brings a unique story, family dynamic, culture, and set of wishes. Learning who they are beyond their diagnosis helps guide care that honors what matters most to them. That kind of care is only possible because each discipline contributes a different perspective, creating a more complete picture of every patient and family we serve.

For Dr. Hernandez, building trust can simply come from speaking the same language. As a bilingual physician, he can communicate with patients and families in the language they know best, creating comfort and connection during one of life’s most vulnerable seasons. Even when a patient can no longer communicate, he says it remains a privilege to be welcomed into such a deeply personal chapter of their lives. He noted this privilege is shared by every member of the care team who walks alongside that family.

“Expectations and culture can have a big bearing on how patients are educated at the end of life. I’ve had a lot of patients who have seemed to be very grateful for me speaking to them in Spanish.” – Dr. Hernandez

Dr. Adams has found that hospice care has transformed his own perspective on life. Supporting families through uncertainty has reminded him to cherish the present and appreciate time with those he loves. Having been born and raised in Modesto, caring for patients in the community that shaped him makes the work especially meaningful. Yet he is quick to mention that the support families receive extends far beyond the physician’s role. Every interaction, every visit, every phone call, and every act of coordination contribute to the experience patients and families remember.

Dr. Lai reflected that this spirit of collaboration is one of our greatest strengths. When every department works together, our mission becomes more than words; it becomes something patients and families can feel. Compassion is strengthened through partnership, and excellence is achieved because each person brings valuable expertise to the journey.

Every department plays an essential role in the care we provide. From Home team, the Alexander Cohen Hospice House, Bereavement, Intake, Medical Records, Volunteer Services, Quality, Finance, Information Technology, the Hope Chest Thrift Stores, and Administration, to all other departments across CHHS, every role helps create comfort, dignity, and hope for the people who place their trust in us.

Together, we are more than individual departments. We are united by a shared mission to enhance the quality of life for all, and we uphold our promise to ensure that no one walks their path alone.
