Cardiovascular Specialty Program
Living with chronic heart conditions can make every day a challenge that can lead to crisis situations, hospitalizations, and unwanted care. The Cardiovascular Specialty Program focuses on enhancing quality of life by reducing crisis situations and helping patients look forward to each day with minimal or no symptoms, worry or stress.
We understand that most patients and their families wish is to remain at home and avoid repeated trips to the emergency room and hospitalizations. Community Hospice & Health Services has developed a Cardiovascular Specialty Program to meet individual needs. Our goal is to take care of patients’ symptoms early and provide education so that they may remain safely at home.
With Our Program One May Expect:
- Consultation, evaluation and care overseen by our Medical Director and board-certified Cardiologist, Dr. Peter Lai.
- Routine visits by a nurse with specialized training in the evaluation and management of cardiac disease.
- Specific education designed for patients and their families managing cardiac disease.
- Medication management.
- Additional supportive care in the form of visits provided by a nurse practitioner, social worker, nurse assistant, chaplain, and/or volunteer at the patients request.
