This Privacy Policy (“Policy”) explains how your personal information is collected, used and disclosed by Community Hospice & Health Services when you access or use our website, located at hospiceheart.org, or when you interact with us offline or on third party websites where we have an official presence. You may opt-out of receiving emails from Community Hospice & Health Services by following the instructions in those emails. If you opt-out, we may still send you non-promotional emails, such as emails about donations or our ongoing relations. Community Hospice & Health Services may update this Policy from time to time. When we do, we will revise the “Last Updated” date at the top of the Policy.
