Community Hospice & Health Services (CHHS) proudly serves a wide region across Northern California. We provide compassionate care in Stanislaus, San Joaquin, Merced, Calaveras, Mariposa, and Tuolumne counties. In addition to these core areas, we also extend our services to parts of Contra Costa, Sacramento, Alameda, and Santa Clara counties. At CHHS, we strive to support patients and their families by offering comprehensive hospice and health services. These are tailored to meet individual needs and improve quality of life.
