Legacy-minded leaders ensure a better future by creating something that outlasts them and enables a long-lasting impact on others. A legacy is a valuable gift to give, which is only possible when leaders are aware of the powerful legacies they have to offer. 

Our Legacy Fellowship was established in 2024 and built many individuals in our community. They have joined together to ensure the patients and families served by CHHS will have critical end of life and other health support services when needed most. 

You may join this Fellowship by making a Living Legacy Gift of $25,000 or more. Your gift can support our work now, either as a pledge paid over three to five years, or as a one-time gift. You will also be invited to the Fellowship if you make a declaration of intent to provide a bequest of $25,000 or greater as evidenced by your will, trust or other declarative instrument.

• Recognition on Perpetual Plaque at the Haig & Isabel Berberian Patient Services Center in Modesto
