
Legacy Fellowship
Legacy-minded leaders ensure a better future by creating something that outlasts them. This enables a long-lasting impact on others. A legacy is a valuable gift to give, which is only possible when leaders are aware of the powerful legacies they have to offer.

Our Legacy Fellowship was established in 2025. This created a giving opportunity for our community. It ensures the patients and families served by CHHS will have critical end-of-life care. Additionally, it provides other health support services when needed most.
You may join this Fellowship by making a Living Legacy Gift of $25,000 or more. Your gift can support our work now, either as a pledge paid over three to five years or as a one-time gift. You may then be invited to the Fellowship if you make a declaration of intent to provide a bequest of $25,000 or greater. This should be evidenced by your will, trust, or other declarative instrument.

It also ensures our ongoing support. The members of the Legacy Fellowship share a bond of commitment to those most in need.
As a member of the Legacy Fellowship you will receive:
- Induction celebration
- Annual reception with a State of the Agency Address
- Legacy Fellowship donor listing in Annual Report
- Perpetual Website Recognition
- Recognition on Perpetual Plaque at the Haig & Isabel Berberian Patient Services Center in Modesto
- You may choose to give anonymously

You may become a member of the Legacy Fellowship through major gifts of planned giving options, such as:
